Funds are awarded to family’s based on the following:
Geographic Location
Recipients must meet one of the following conditions:
Reside within Lewisboro Township – hamlets include Cross River, Goldens Bridge, South Salem, Waccabuc, Vista and Lewisboro.
Be receiving medical care for the child from an approved facility within the Tri-State area – NY, CT, NJ.
Need-Based Qualification
Financial assistance will be provided to families based on the following factors:
Medical Necessity: Child diagnosed with a life-threatening illness by a licensed medical professional.
Financial Hardship: Demonstrated inability to cover necessary expenses related to care, assessed via:
- Statement of hardship letter and/or referral from a hospital social worker or care team (supporting need based on criteria such as, but limited to, income level, existing medical debt, other financial obligations, etc.).
Lack of Alternative Support: Family does not have sufficient insurance coverage or other financial assistance options
Covered Expenses
Funds may be used for the following essential expenses directly related to the child’s care:
Medical Costs:
Hospitalization and medical procedures
Medications and treatments not covered by insurance
Specialized medical equipment and supplies
Transportation & Lodging:
Travel costs for medical appointments (fuel, public transit, flights)
Temporary lodging near hospitals or treatment facilities
Caregiver Support:
Home healthcare services
Respite care for parents or guardians
Counseling and mental health support
Other Essential Costs:
Funeral and end-of-life expenses in the unfortunate case of the child’s passing Adaptive modifications to home (ramps, accessible facilities)
Child-specific educational support related to their medical condition
